We are so excited that you are interested in volunteering in PCC Kids!

To help us best serve you, please complete the form below. This form is for all volunteers involved in the supervision and teaching of minors in PCC Kids. Information will be treated as confidential and is needed to help us provide a safe environment for children.

***Please note, completing this form may take you 15-20 minutes. Also, the form concludes with a request for 3 references that have known you for at least 2 years. Please have these references ready prior to completing this form. References should not be relatives (if you are under the age of 18, you may use a parent and/or teacher). If possible, please include at least one reference from someone at PCC.
